I Am A 50 Year Old Male Transgender In Reletively

I am a 50 year old male transgender in reletively good health and about three days ago i have had a dull to sharp pain in my right testes. There is swelling and it feels like some one has kicked me in the "balls". At first i thought it was a hernia, but a female friend of mine told me that it sounds like i have tubelar torsional something what is that and how did i get it and is there anyway to reverse it. I have put cool towels on it and even applyed mild pressure to push it around and furthermore they are not "hanging" low as they should be the right one is hugged up very close to my body and the they are not "cooling" as they should be right now the pain is gone because i have twisted it (although very slowly and painfully) but it did provide some relief. HELP!, TRANNY IN DISTRESS!!!
