I Am A 58 Year Old Male. 15 Years Ago

I am a 58 year old male. 15 years ago I had C5, C6, and C7 fused. Now I have pain in the left shoulder, left tricep, left forearm, and hand with resting tremors occuring in the left arm. I have numbness in the last two fingers of the left hand. The right shoulder has some pain and I get numbness in my right hand using a mouse for an extended period of time. I also have head aches at the base of my skull and behind my left eye on the left part of my head. My health in the last 3 years has been poor as I have had toe amputations, charcot surgery, double bypass surgery and MSSA and MRSA infections. Is the pain something I will have to bear or is there some avenue I should look into. It effects my sleep, work and leisure.
