I Am A Social Worker Working With A Young 16

I am a Social Worker working with a young 16 year old girl who has recently told me she has been hearing voices. She has been on 75 mg Zoloft for quite a few months. Since she has been placed in treatment foster care the primary doctor added 2mg of Abilify and dropped her Zoloft to 50 mg. She also recently increased her Abilify to 4mg. She takes her Abilify at night before bed. Her voices are during the night. She is very restless and very impulsive on top of the voices. She said she experienced voices before which her father confirmed when she was prescribed Lamictal and Remron combination. This was d/c. She has a diagnosis of bi-polar but seems to have traits of ADHD. She does not sit still long enough to have a good conversation...then will get argumentative from asking her questions. She has SIB in the past and has very delinquent behaviors which are very risky behaviors, alcohol, drugs and runaway behaviors. My question is do you think the Abilify is causing the voices and if so do you have any recommendations. I am hoping to get her assessed soon. I know there is a lot going on with her but the voices are concerning to me.
