I Am Currently Taking Eliguis, Diltiazem, Hydrochlorothiazide And Omeprazole Medications.

I am currently taking Eliguis, Diltiazem, Hydrochlorothiazide and omeprazole medications. I have developed a rash and itch for the past year. Which medication is causing this? Or could it be a food interaction with one of the meds that is causing this?
