I Am Having A Problem With A Double Stream In

I am having a problem with a double stream in my urine. I having discomfort with my right testicle for quite a while, but recently it has gotten worse and is painful all the time. Seems to be enlarged from what it was. I am 80 years old. 8 years ago I had my prostrate remove due to cancer. Then I had to have bladder repair because the surgeon had put a hole in my bladder. Could this be caused from back then. Do I need to get a antibiotic.
I am vacationing in Florida right now and my urlogisist is back home in Michigan. What kind of insight can you give to me on this?
