I Am Having Discharge From My Nipple Which Is Causing

I am having discharge from my nipple which is causing it to itch and bleed if I scratch it. I am over 40 years old, don't have kids that I am breast feeding, been going on for a couple years, get it healed and it keeps coming back, don't have medical insurance, so I cant afford to go to doctor. I don't know what to do?
