I Am Looking For The Names Of Primary Care Physician

I am looking for the names of primary care physician for my children ages 5 and 19. It is important at this time that I have access to specialist at north shore hospital in evanston as they will both need to be referred to specialists. We live in evanston and the children have medicaid.
