I Am Sugar Patient Last 18 Years. Still A Day

I am sugar patient last 18 years. Still a day I take medicine glycomit sr per day 1850 gm and insulin humalog 50 mix . Morning 40 unit, evening 16 unit. Yesterday I will test my blood sugar result fasting is 177 and pp is 236,hba1c 6.8, micro albumin 15.31, creattinine 1 , but sgpt alt is 351 and sgot ast is 106 .please give me solution.
