I Am Taking Hydromorphon 6mg Twice Daily And 1mg Delaudet

I am taking hydromorphon 6mg twice daily and 1mg delaudet as needed for chronic back pain. Hydromorphon don't work anymore for pain but I have a terrible side effects, difficult breathing, slow hearth beat, nausea, I can't eat. I have to stop taking this medication. Can you help me? I take high blood pressure pills and I am diabetic.
