I Believe I Injured My Right Shoulder Holding My 5

I believe I injured my right shoulder holding my 5 mo old 18 lb grandson in my rt arm. I can reach around my back with my left arm to mt rt side, but with my rt arm I cannot reach around my back at all. It just won't go. Very painful. Does it sound like rotator cuff tear?
