I Broke My Ankle/foot About 20 Years Ago & I

I broke my ankle/foot about 20 years ago & I have flat feet...about a year ago I noticed mild swelling in that ankle that goes away with rest & elevation. Recently, my big toe is getting the pins & needles &my ankle is swelling to the size of a grapefruit & it hurts really bad. No bruising. Range of motion not really limited. x-ray showed "nonspecific swelling". Lab work came back normal. Not diabetic. The pain is excruciating...especially after being on my feet for 12 hrs at work. I have to take ibuprofen just for the pain to ease up enough for me to get to sleep. What should I do?
