I Found A Calcified Lesion Posterior To The Medial Sacral

I found a calcified lesion posterior to the medial sacral base (at least 3 inches posterior), on a lateral lumbar radiograph today. Patient presents with severe sciatica of 2 years duration, progressively worsening.
What is the differential diagnosis and what is my next base diagnostic step. (35 year old female)
