I Had L5S1 Laminectomy Fushion Surgery Four Years Ago.. Most

I had L5S1 laminectomy fushion surgery four years ago.. Most of the symptoms have returned i.e. Low back pain pain down the buttock to the leg to the foot. Is this known as failed back surgery or could there be further damage. I have resumed pain management which are a variety of shots and still not much relief. Am I missing something?
