I Had A Hysterectomy Nearly Five Weeks Ago And I

I had a hysterectomy nearly five weeks ago and I have been having blood on the toilet paper when I wipe only. I am assuming this is because of blood thinners I am on but I'm also having a yellow browny watery discharge. However over night I woke to find that I had gone way through my underwear. Any idea why this might be happening?
