I Had A Major Surgery In 2008. Laminectomy And Fusion

I had a major surgery in 2008. Laminectomy and fusion at S1 - L5 - L4. All was good until I fell in 2009. Fusion broke, screws loosened. By the time I got to my surgeon, the fusion started to reheal. Have been dealing with lower back pain ever since. Over the past month, I have gotten shooting pain down the back of my legs. This is brought on by everything from walking to just moving slightly in a chair. Even blowing my nose will bring the pain on. Any chance that this could be a hip problem, or is it my back again?
