I Had A Skin Graft Done On My Hand Due

I had a skin graft done on my hand due to a third degree burn and cold weather makes it hurt terribly. It has been more than a year and a half since the graft. It aches in my fingers as well. What do you suggest I do or what kind of doctor do I see for this?
