I Had A Very Bad Fall While Riding My Gazelle

I had a very bad fall while riding my gazelle (exercise machine). My upper rear end landed on the back bar and caused extreme pain the next day the bruise was huge. While the bruise has disappeared I know have numbness where the bruise was and a dent with pain that aches deeply . I have noticed a pain shooting down my leg as well. Just wondering if I should be concerned?
