I Have Lupus And RA. I Would Call Them Severe

I have Lupus and RA. I would call them severe as i never go into remission and am in pain all day and run fever. I get really red and swollen with heat at nearly all my joints. I have been having shoulder pain along with the redness and heat for about a year now. I have it in both shoulders, but the left shoulder is the worst by far. I have had several steroid shots in my shoulders but it did nothing for the pain. I recently had a MRI of the left shoulder and just got the results in the mail. I have an appointment to see a orthopedic surgeon on the 28th but this pain just keeps getting worse. I also have lost a lot of strength in my arms and cannot hold my newborn grandchild. I am 51. These are the results of my MRI Impression:
1. Interior likely superior slap tear with suboptimal evaluation of the labrum due to motion and positive intra-joint fluid.
2. High grade partial thickness tears of the infraspinatus and supraspinatus with component of full-thickness tears.
3. Likely longitudinal split tears involving the intra-bicipital groove bi-ceps tendon

My question to you is can you tell me what the above means? Did I injure my shoulder and not know about it? Do you think this will require surgery? Will it hurt my shoulder to wait this long to see a doctor much less to have surgery? Any help would be greatly appreciated.
