I Have A Prince Albert Piercing Which I've Had For

I have a Prince Albert piercing which I've had for over 10 years with no problems.. The skin just below the head on my circumsized penis keeps splitting for the last few months in the same spot near the piercing.. Neosporin seems to help it heal pretty well then anytime I have intercourse, especially with a condom, it splits again and it very painful. It seems to be developing scar tissue now. could it be a yeast infection?
