I Have A Combination Neck Pain And Shoulder, Upper Back

I have a combination neck pain and shoulder, upper back pain. When working out, the neck pain occurs as I am laying on my back and either laying my head down on the mat, or lifting my head up. When sitting or standing, if I tilt my head or neck back, I get pain in my upper back on the right side above shoulder area. I feel it at what I think is some kind of nerve at the base of my neck - again at the middle to right side. Finally, this is connected to pain, usually on my right side at top of right shoulder down to what I have heard is called the scapula or scapular region. Again, right side. Sometimes I can ease it by a sort of massage or push in the front above my chest to the spaces between chest and shoulder.
