I Have A Constant Dull Sickening Pain In My Right

I have a constant dull sickening pain in my right testicle for about 3 months ...also have an extreme itch in lower scrotum...that can only be stopped by sever scratching and or pinching very hard...breaking the skin...I've tried Nystop powder and every kind of ointment...nothing works and it keeps getting worse...I shower every day and have tried different soaps as well as no soap....I have hygiene practices...and I had jock itch in high school...but this is not the same....between the two problems it's about to drive me nuts...no pun intended...but seriously I can hardly stand it anymore...what do you think...?
