I Have A Constant Infection. It Usually Starts With Ringing

I have a constant infection. It usually starts with ringing in my bad ear then travels down my neck to other weak spots like my herniated back or my stomach. Pain a and misery. Then I get an antibiotic and it goes away for a year or so as long as I keep my head above the water I work in. this year the infection is stuck. Took ciproflaxin and got the pain to go but the ringing never quit. Now everything seems to be infected. From my ears to my thick green toenails. Do I NEED A MORE POWERFUL ANTIBIOTIC or something else. Now my throat hurts and it keeps me from getting sleep.
