I Have A Dear Friend Who I Suspect Is Ill,

I have a dear friend who I suspect is ill, but he refuses to seek medical attention. I want your ideas as to what may be wrong with him. He is 69 year old white man, thin,6 ft tall smoker. He is outside often and lives in an urban area. He has a prominent bulge in his crotch area and it has been obvious for about a year now. He recently bent over and could not get upright. Now he is telling people about what to do in the event of his death. Does this sound like it could be a serious condition? It's there something you recommend I can say to help him?
