I Have A Finger That Is Swollen At The First

I have a finger that is swollen at the first joint and the fingernail is black and falls off . I have had this problem for 5 years now and no one can tell me what is going on. I have taken anti fungal and anti anti inflammatory. and nothing works. I have also taken gout meds and that didn't work either. I can not move the finger and it is swollen and hot and hurts all the time. can you help me or tell me what kind of DR to see. thank you nette
