I Have A Fistula That Has Not Been Used Since

I have a fistula that has not been used since my transplant five years ago. The Dr decided to leave it in since they had to use my brachial artery. Now the thrumming has stopped and the arm is swollen, red and very painful to use. What could be the cause for the symptoms?
