I Have A Friend Whose Husband Is Tired, Really A

I have a friend whose husband is tired, really a lot more than just tired all the time. He's diabetic and high blood pressure. He just recently had a heart cath and everything was ok, blood sugar is also fine. His doctors can't seem to find anything, so what would be your next step?
