I Have A Swollen And Painful Vain Under My Right

I have a swollen and painful vain under my right armpit area. Comes and goes in severity for a few days now. I'm thinking maybe a swollen lymph node is to blame but not sure. I say this because at 45 I am 7 weeks into HRT for MtF transition as a transgender woman. Could that be it and should I be worried? Thanks.
