I Have A Very Painful Toe Joint That Has A

I have a very painful toe joint that has a raised red pointy lump on the top of my foot. I suspect it's an osteophyte. I had them dealt with 20 years ago by surgery and fusing the toes bones. Why has it 're-occurred and will surgery cure it. I can't wear ordinary shoes for the pain.
