I Have An Anal Fissure For Many Years. But Last

I have an anal fissure for many years. But last month suddenly it was bleeding badly. Then I consulted with doctor and was in medication. I am using anovate cream for last one month, the result was good and again in last 2 days it started bleeding. I am not sure why it happened again and what to do next
