I Have Been Around Smokers My Whole Life So I

I have been around smokers my whole life so I am not ruling out any options. I have been sick for close to a month... if not longer. Have lost my voice for a few days and have had a cough the whole time and it seems to be getting worse. I have also been having sever chest pains... doubled over and unable to breath. Went to the doctor and was told I was fine. They did EKG on my heart and X-ray on my lungs said everything was good. I also get bad right shoulder pain...even when I'm just standing there talking to someone. They usually happen together... chest pain then shoulder pain or shoulder pain then chest pain.
