I Have Been Diagnosed For The Second Time With Renal

I have been diagnosed for the second time with renal cel carcinoma, I will be scheduling surgery on Wednesday. They are taking the whole kidney this time, last year they did a partial nephrectomy. It was stage one and hadn't breached encapsulation, best possible outcome. However now a year later its back. Also on CT scan is something on spleen, they will look into while I'm oin operating table, and three spots on my lung, too small to distinguish its origin, I do have a history of crohns with lung involvement aka pirodermagagnurosum. My question is can renal cel carcinoma spread to spleen, and how big do the spots have to be on lung for a pet scan or biopsy. My piroderma was diagnosed with an open lung bioopsy , can I avoid that? Thank you
