I Have Been Having Muscle Tightness In My Chest, Upper

I have been having muscle tightness in my chest, upper back jaw, neck and upper arms Accompanied by weakness in legs, pain when swallowing, headache. I also experience a smell of smoke when having these episodes. Have tried migraine meds to to relief. Am 45 year old female. These episodes seem to be triggered by physical or mental fatigue. They last a few hours. Afterward I feel fatigued and weak. I also feel as if I have something stuck in my throat when this happens. Any suggestions
