I Have Been Struggling With Concentrating At Work & I

I have been struggling with concentrating at work & I lost my last job because of this. My current manager has approached me recently about this & I'm honestly scared this is going to cause issues for me again. Any help would be appreciated because I desperately don't want to lose another job.
