I Have Been Treated By Several Doctors For What They

I have been treated by several doctors for what they have now diagnosed as osteoarthritis of both knees and my lumbar spine (with disc degeneration). I feel I have received "piece meal" treatment and am looking for a physiatrist who will offer multidisciplinary care, but when I ask various practices about this kind of care they talk to me like they have no clue what I'm talking about. Do you know why this is and what you would suggest regarding treatment for what I mentioned above plus other conditions I have that are all related in some way: chronic daily migraine, depression, lymphedema, gait and balance issues....???
