I Have Been Very Hoarse, With Almost Loss Of Voice,

I have been very hoarse, with almost loss of voice, for about 9 months. Strain to talk. When I lean back against chIr or anything there is a spot in my upper right back that causes an appearance of a knot or something coming in my throat on the right side. All tests come back negative. Any idea what is going on? Thank you
