I Have Central Stenosis Secondary To Of Focal Central Protrusion

I have central stenosis secondary to of focal central protrusion on the background of a diffuse disc bulge at L3_ L4. And centralprotrusion is also seen involving the L4_L5 disc compressing the ventral thecal sac and causing mild central canal stenosis at this level also ligamentum flavum thickening is seen at both levels. Focal hemangioma is seen within the L4 vertebral body do you think this requires surgery
