I Have Chronic Constipation And IBS. I've Struggle To Pass

I have chronic constipation and IBS. I've struggle to pass a bowel movement for a few weeks now only getting tiny, hard and dark stools on the odd occasion I pass one.
I believe I still have quite bad haemorrhoids and the anus is swollen and tissue tends to poke out just a little when on toilet, coughing/sneezing and now sometimes during movement/exercise. It's sore like a burning/graze pain. Alongside severe pain in lower back and lower abdomen.
I am even struggling to pass wind.
There has also been mucus like discharge which is always coming out.
Please help? What should I be asking my doctor to check when I see her?
