I Have Chronic Hives I Broke Out Off And On

I have chronic hives I broke out off and on for 3 months. I have eliminated and changed foods, detergents, went through meds one by one to no avail. What about insulin I have taken the same insulin for 20+ years, I recently saw I was using a very outdate bottle of Humulin R, could this cause hives.
