I Have Congestive Heart Failure And Coronary Artery Disease...have Had

I have congestive heart failure and coronary artery disease...have had several stents and blockages. I also have a hiatal hernia and acute esophagitus (?). However the other day, I was in a hospital with a friend and had all the symptoms of a full blown panic attack. How can I...or a doctor....determine which of these things occurred? I had an angiogram 3 days ago and they said there was not any significant narrowing of the arteries at this time. But what I felt sure felt like a heart attack. Shortness of breath; a cold sweat; one gigantic pain in the very middle of my chest and accompanying pain in my left arm and jaw. I also had an endoscope yesterday which showed the hiatal hernia and enflamed esophagus. How do I tell what is what?
