I Have Dishydrotic Exzema On My Hands For Over 30

I have dishydrotic exzema on my hands for over 30 years and have been receiving triamcinolone acetonide injections quarterly since with varying degrees of relief. Now I have osteoarthritis with osteophyte formation and the doctor gave me a intracapsular injection of triamcinolone acetonide to help with inflammation. It helped with my shoulder and systemically with the exzema. Two birds with one stone as it were. Now I see an over the counter medicine Flonase , triamcinolone acetonide 55mcg. / spray. Would it be safe to use that? Would it work systemically for my two conditions as an off label therapy?
