I Have Dry Skin Around Head Of Penis And Is

I have dry skin around head of penis and is very itchy every once in awhile i am 15 and shaft of penis gets dry also i also have shiny like flat spots on testicles and is itchy sometimes i do masturbate could this be from it ? Or is this some type of disease or what im worried and need help
