I Have Had Hemmroids Lately But Now I Have Itching

I have had hemmroids lately but now I have itching and three bumps forward from the area of the hemmroids. I am not sure if hemmroids can spread or if it is a different issue. I am using preparation h but I took a spinning class today and the itching is very aggravating. Not sure which type of Doctor to call or if soaking with Epsom salt would help?
