I Have Had Leg Pain And Numbness On And Off

I have had leg pain and numbness on and off for about 7 years. I had an MRI and it shows two herniated discs, spinal stenosis, radiculopathy, arthritis, etc. I tried chiropractic care, physical therapy twice, with temporary results. I have had two spinal injections (L4-L5) over the past four months. I am scheduled for one more this Friday. The pain is so intense in my leg I am walking with a cane and have to crawl upstairs. I have always been very active, have weight trained for over 30 years, and now believe that caring for my aged mother and working at a computer 45-50 hrs a week have done me in. Should I ask about spinal surgery? I refuse to take pain meds other than Ibuprofen, but am able to sleep after a few hours if I take Gambapentin. Please help, I feel so hopeless.
