I Have Had Numbness In My Toes For About A

I have had numbness in my toes for about a year and now it's starting to spread kind if fast up one of my feet, I also have an itchy spot on the top of that same foot located just a little above my numb big toe. It kind of looks like a nerve. What could be wrong with me? I'm only 21.
