I Have Irregular Menstruals. Recently I Had One That Lasted

I have irregular menstruals. Recently I had one that lasted 45 days it stopped for 12 days and then started back on December 24th 2016. This time it has been really light which is abnormal for me because usually I bleed really heavy when I start bleeding but I really haven't had anything. Just little spurts of blood coming out here and there. I was using the equate version of pads. The thin overnight ones bc i am a bigger female. Recently I had to stop using them because I started itching and now the outside is raw and swollen. This has happened once before because I bleed too long. Last night (1-4-17) i was cleaning myself so that I could put some diaper rash ointment down there and I noticed a few things. One thing I noticed was that I had this little piece of skin hanging out from my vagina it was attached and kind of like a oval shaped. I also noticed that what look like blood clots that were Dry and elastic was also coming out. These blood clots feel like skin like the lining was coming out but they only issue is is that they're grey and don't look normal at all.
