I Have Just Been Discharged From Hospital But Still Feel

I have just been discharged from hospital but still feel extremely poorly, my DDimer test was elevated at 2671!!! I have had a CT scan and a scan of my left leg which were both clear. I initially got referred immediately during a visit with my GP as my BP was 220/134, heart rate of 136, I was grey in colour and facially sweating with chest pains and shortness of breath. I am 44 year old female medium build. Since being discharged I still have shortness of breath, and extreme tiredness, I am really concerned as all I am doing is sleeping, and no appetite.
