I Have Left Sacro-iliac Joint Pain, Left Groin Pain And

I have left sacro-iliac joint pain, left groin pain and left testicular swelling with tenderness. No signs of infection (no nausea, vomiting, fever, exudate, normal lumbar spine x-ray). A course of Ciprofloxcin was started 11 days ago with no relief. Heat helps the lower back pain and support to the testicle adds comfort. Left testes is about twice the size of the right. No exterior signs of torsion (pain has not increased or decreased with time, no bruising or discoloration or necrosis). Any ideas? I have not ruled out inguinal hernia but have no palpable protrusion noticed in the left lower quadrant or scrotum. Thanks
