I Have Pain Usually Worse In My Hands And Feet

I have pain usually worse in my hands and feet (toes and ankles). It is not a constant year long pain.. maybe happens 3-4 times a year. It may start in only one or two toes and when they start to feel better it seems to move to my hand and wrist for maybe a week and then it will move to my other hand. The pain is very terrible and almost unbearable. It also has extreme swelling and a fever with the pain. When it is in my hand I can't close my hands at all without severe pain. I can put my hand under fairly hot water and I can close my hand with no pain but a heating pad doesn't seem to help at all. Have tried heat and cold and neither seem to help. Have been dealing with this problem for almost 15 years and its getting worse now. It usually lasts about a week or two and then goes away like nothing ever happened. Have had tests ran and Doctors just tell me its not gout or arthritis. Wish someone could just tell me what it was and give me something for the pain and put a stop to it. It really effects my life in many ways from work to family. Thanks
