I Have Recent History With Blood Clots And Had PE

I have recent history with blood clots and had PE in June. Had a minor accident in July and have been off blood thinners since July. I am feeling so tired and winded doing minor household tasks. As of yesterday I have had pain in left shoulder area, pain not major but I feel twinge with deep breath. I lost my job and health insurance in August and I have no money. I am loath to go to ER unless life and death. I am so tired. I don't know what to do.
