I Have Several Conditions....CHF, COPD, Diabetes, Wegener's,, A-fib...lately Whenever I

I have several conditions....CHF, COPD, Diabetes, Wegener's,, A-fib...lately whenever I walk a short distance, the whole upper part of my body kinda "seizes up", and then my legs become weaker than usual. If I rest for a minute, I return to normal. I'm not sure which condition is causing this?
